Below is a sample claim letter designed for seeking payment for unpaid Extra Trips that have been ordered under PS Form 5397.  You will need to insert the date, the name and address of the contracting officer, and the total amount of the claim.  (You must state an exact amount.)  The PS Form 5397s should be attached to the claim letter.  It would also be helpful to provide a spreadsheet or list showing the date and amount of each unpaid Extra Trip, and total amount. 

The claim letter and attached Extra Trip forms should be sent to the Contracting Officer (you can also send to the Administrative Official, but only the Contracting Officer can issued a decision).
